Oklahoma’s legislation requires Form 4 to be utilized in the following situations:

1. Before gas is run to a new system for the first time
2. If 2 years have passed since a previously serviced system has been operated
3. When new or modified gas piping is placed into service
4. Any time a gas leak is suspected
5. When an interruption of service occurs, including a gas runout
